At Fosse Dental Care we are currently accepting new NHS patients. If you would like to become an NHS patient and you are 18 or over you must attend the practice with the following documents (we do not need to see any documents for under 18's);
 
1. Proof of address
2. Photo ID Proof of benefits (If you receive them)
3. NHS Medical card so that you can provide us with your NHS number, if you do not have this you can contact your GP for this information.
